We begin with a thorough review of your agency's past surveys (initial, complaints, and/or re-licensure) to understand your agency's specific needs and goals. Your Consultant will review results from your last audit and create an action plan for review.

Our team will arrive onsite (for 5 - 7 hours) to review your agency's documentation, including policies, charts, meeting minutes, etc. Our experienced consultants will also provide onsite training, for continued compliance with DOH regulations.

You will receive a comprehensive report outlining any discrepancies found and recommendations for corrective action. We will work closely with your team onsite to implement the necessary changes, providing guidance and support every step of the way.
REVIEW DOCUMENTS
Policy and Procedure Manual
We will review your policy and procedure manual to ensure it is compliant with DOH regulations. In the event your policy is not up to regulation standard, we will provide a fully updated policy manual for an additional fee.
Sample of Personnel charts
We will review a sample of personnel charts to ensure they are compliant with DOH regulations. This includes providing your agency with updated templates that meet regulation standards.

We will review a sample of patient charts to ensure they are compliant with DOH regulations. This includes providing your agency with updated templates that meet regulation standards.
Quality Assurance Meeting Minutes
We will review your agency's QA meeting minutes to ensure compliance. We will provide detailed reports of the internal chart audits for your next QA meeting for review.
Governing Authority Meeting Minutes
We will review your agency's Governing Board meeting minutes to ensure compliance. We will provide detailed reports of the internal policy audits.
Complaint Log
We will review your complaint log to ensure compliance with regulation standards and include templates that meet regulations.
Emergency Preparedness Plan
We will review your emergency plan and include all DOH mandated components including policy, templates, rosters, and drill documentation.
Home Care Registry Review
We will assist with cross-referencing your personnel rosters with the registry to ensure compliance.
Communications Directory Review
We will create all necessary rosters, along with staff call-down list and organizational charts. We will then cross reference all information to ensure the directory is up-to-date.
Criminal History Roster Review
We will assist with cross-referencing your personnel rosters with the CHRC roster to ensure compliance.
Rosters Review
We will create all rosters necessary for operating your LHCSA.
Follow Up and Collaborative Approach
You will receive a comprehensive report outlining any discrepancies found and recommendations for corrective action. We work closely with your agency to implement the necessary changes, providing guidance and support every step of the way. You will have the option to book another review in 90 days.
