HOW THIS WORKS
1. Distribute the flyer below
Please see attached flyer (to print and distribute to your Nurses and Administrator staff for them to scan with their mobile phone).
2. Email log-in link to caregivers
You also have the option to copy and paste this link and email it directly to caregivers to register for their own log-in credentials: https://form.jotform.com/240074839114050
3. Receive user log-in details
After completing the form (either via flyer or email link), caregivers will automatically receive an email with user log-in details.
4. Caregivers have 12 months to review content
Caregivers have up to 12 months to watch all video content inside of the platform to meet DOH regulations for compliance. Caregivers will receive monthly emails as a reminder to complete the in-service video course.
5. Course certificate
After completion of the course, Caregivers will receive a customized certificate. Caregivers are responsible for emailing the completed certificate to the agency Administrator or Coordinator to place in their personnel charts.
6. Administrator notifications
The Administrator will get a monthly email updates about the status of your staff's completion.
